I, the undersigned, have read, understood to my full satisfaction and completed the PAR-Q+ questionnaire. I acknowledge that this physical activity clearance is valid for a maximum of 12 months from the date it is completed and becomes invalid if my condition changes. I also acknowledge that the community/fitness center/trainer may retain a copy of this form for records. In these instances, it will maintain the confidentiality of the same, complying with applicable law.

Assumption of Risk: I acknowledge that participation in physical activity involves known and unanticipated risks which could result in physical or emotional injury, paralysis, or death. I understand such risks cannot be eliminated without jeopardizing the essential qualities of the activity.

Release and Hold Harmless: I hereby voluntarily release, forever discharge, and agree to indemnify and hold harmless OceanWavesFit, its officers, employees, and agents from any and all claims, demands, or causes of action which are in any way connected with my participation in this activity or my use of OceanWavesFit's equipment, facilities or programs, including any such claims which allege negligent acts or omissions of OceanWavesFit.

Indemnification for Legal Costs: Should OceanWavesFit or anyone acting on their behalf be required to incur attorney's fees and costs to enforce this agreement, I agree to indemnify and hold them harmless for all such fees and costs.
